Question OBD2 - P0420 Diagnosis

Today my 1997 D90 threw a "P0420" “Catalyst system efficiency below threshold” (Bank 1) code. It threw the same code exaclty 60 days ago... so whatever the problem is, it isnt going away.

My research on the web indicates any of the following causes:

An oxygen sensor is not reading (functioning) properly
The engine coolant temperature sensor is not working properly
Damaged exhaust manifold / catalytic converter / exhaust pipe
Retarded spark timing

Anyone have experience with this code? What was the resolution?
