Engine and Transmission question

I have a D110 300TDI with an R 380 Transmission. when I am in 5th gear I hear what sounds Like a Tic Toc coming from the front. It is louder when i accelerate and sort of gos away when I rake my foot off the gas. It does not occur in any gear other than 5th gear. Anyone have any sugesstions on what could be the problem. Shawn

Is the tic toc sound rapid (piston/crank rate) and does it vary with rpm?

It does not make the noise in reverse. On my way home from work today it was constant at 55 to 75 miles an hour but if I let up on the gas then the noise would subside some but only in 5th gear.
