Do I have a clutch issue ? Or is this something really bad

Having some problems with the truck when getting into 1st gear. Time of day not an issue, first starting up, be it in the morning or end of day. When shifting into first gear and left foot is coming off the clutch with right foot coming down onto the gas, I have a shudder in the engine. Its as if not enough gas is going in or I'm off the clutch to fast. if I over rev the engine, no shudder buy RMPs have to be way up there.
Funny thing is you get a feel for gas/clutch and 1st gear is very different. I'm not feeling or sensing it anywhere else, any other gears just 1st. I had the 4th / 5th gear sync go last year and the truck is a 1994 which just rolled over 92k. I've had the truck since 64k and other than a master cylinder no issues in this area.
So what's the prognosis, Clutch slipping or wearing out, or...?

Do you have a really leaky rear main? oil one the friction disc can cause them to be "grabby"

Checked the back and the levels. all good. Getting the same thing in reverse.
