96 D1 o2 sensor issue

I have a 96 D1 here with all 4 O2 sensors at 5 volts. I've spent a lot of time chasing what I thought was electrical, but was wondering if this isn't the "effect", not the "cause"

Thoughts on what would cause such a rich condition?
