take your boost hose off from the turbo and check for turbo play. there should be very little. if you are hearing grinding, that is the biggest concern. grey/blue smoke is what you would see if a seal has blown. nonetheless if you have play in the impeller shaft and/or you can see wear marks on the walls where the impeller is rubbing, you are on a countdown to catastrophic failure. at this point, your turbo can probably be rebuilt. if you let it seize up, you may be looking at a full replacement which is more expensive.
there is no reason to modify from a stock setup. a turbo is a part, and it wears over time. just don't delay.
