Does it make a noticeable difference on a Defender?

I don't think they do anything, but it will certainly do nothing on the rear of a Defender. They are intended for axles with radius arms where the pinion angle is different than the transfer case output.

Lost mine on a corrugated road somewhere in Belize. Heard a load thump underneath and looked in my rear view to see what I'd hit to see it rolling trying to keep up and losing ground fast.

I drove back from Belize without it and never really noticed a difference.

Yep, still have it on my D1 axles under my 109. Took it off once and really noticed additional vibration. Put it right back on.
